Invitation to use Advance Care Planning Online Module
Dear Friends and Colleagues,
To support universities and colleges adjusting to online academic environments, the University of Southern Indiana Geriatrics Workforce Enhancement Program (GWEP) team has developed an online module on advance care planning (ACP) for instructor use with students.
The module was designed to be suitable for any future health care professional in your undergraduate and/or graduate courses. It includes multi-media tools for distance education, including a lecture, online film and video clips prepared by content experts, reading material and suggested homework and reflection questions.
The ACP module is available in a YouTube format for dissemination and use by instructors. This is accompanied by an instruction guide that provides suggestions on use of the materials and additional source material.
We ask that each instructor interested in using the module complete this brief request form that will allow us to track usage and report to HRSA on the future health professionals engaged in learning about these topics. A USI GWEP team member will send you the module link and instruction guide once we receive your request in the form of this brief survey.
